Nevada Kidney Disease and Hypertension Center (NKDHC) is one of the largest Nephrology clinics in Nevada. We are considered experts in the treatment of Chronic Kidney Disease (CKD), End-Stage Renal Disease (ESRD), Acute Renal Failure (ARF), Kidney Transplantation, and creating and maintaining access to dialysis therapies. Our company is a professional medical association comprising physicians and other essential support staff. We are expanding and seeking Medical Assistants and Patient Care Coordinators to join our teams in the Las Vegas and Henderson areas.

As an Embedded Resource Specialist, you will monitor and coordinate patients' treatment plans in furtherance of the Treatment Program. You will reinforce education about their condition, ensuring patients are informed and empowered. You will also coordinate care with healthcare providers and partners, facilitating seamless communication and collaboration to optimize patient outcomes. Your role will be pivotal in supporting the overall treatment process and ensuring all necessary resources are in place for effective care delivery.
To be successful, you should have:
- High School Diploma required and 2–4 years of customer service-related experience in a medical office role.
- Medical Assistant (MA) Certificate is desirable.
- Licensed Practical Nurse (LPN) strongly preferred.
- Proficient in medical office software applications, including EMR systems, scheduling programs, and office equipment.
- Your responsibilities as the Embedded Resource Specialist will include:
- Engage with and follow up on CKD and ESRD patients to ensure their needs are met and schedule future meetings.
- Build relationships with patients and collaborate with interdisciplinary teams (nephrologists, nurses, pharmacists, etc.).
- Partner with Optum and Provider resources to manage care, appointments, and care gaps.
- Provide in-person and telephonic interventions to ensure adherence to care plans and resolve any barriers.
- Coordinate care across the kidney care continuum, including scheduling appointments, referrals, and ensuring timely follow-up.
