Lead Care Manager

This full-time, in-office position offers a salary of $26.56 to $33.00 per hour, commensurate with experience, along with a supportive benefits package that reflects their commitment to employee well-being and long-term growth. To succeed as the Lead Care Manager, you should have:
- Bachelor’s degree in Psychology, Social Work, or related field with relevant experience; MSW preferred.
- At 3 to 5 years of experience supporting behavioral health or CalAIM populations, including individuals with complex medical or social needs.
- Experience working with youth and families is strongly preferred.
- Solid proficiency in EHR documentation and comfort using systems such as Exym, Welligent, or Epic, plus dependable internet access.
- Solid communication skills in English; bilingual required- Spanish preferred or other languages welcome. (Please ensure this is included on your resume).
- Valid California driver’s license, insured vehicle, and ability to travel for community-based member support.
Your responsibilities as the Lead Care Manager will include:
- Conduct outreach, enrollment, and comprehensive assessments—including behavioral health screenings and social determinants of health.
- Manage a caseload of 30–40 members, developing individualized care plans with SMART goals.
- Coordinate care across hospitals, affiliates, and community partners, including discharge planning and closed-loop referrals.
- Support members through appointment accompaniment, treatment adherence monitoring, and health promotion/self-management coaching.
- Participate in regular case supervision, multidisciplinary case conferences, and ongoing training to ensure high-quality, integrated care delivery.
