Chronic Care Navigator - AKDHC
The Chronic Care Navigator is responsible for coordinating the care of patient with chronic nephrology conditions, ensuring comprehensive management, support, and education to optimize patient outcomes. You will work to identify patients’ unmet needs, engage patients in their own care, gather information for treatment interventions, and enhance ongoing communication between the patient and their care team.
Essential Duties and Responsibilities: Include the following. Other duties may be assigned.
- Identify and engage patients with chronic conditions. Address gaps in care and unmet health needs.
- Efficiently and effectively manage a panel of up to 500 patients, with a goal of 300+ monthly billable patients.
- Gather, review, and summarize patient data to support pre-visit planning, treatment, interventions, and real-time clinical decisions.
- Partner with nephrologists and interdisciplinary teams to improve care continuity and population health outcomes.
- Conduct outreach calls to assess needs, educate patients, and reinforce care plans and medication adherence.
- Accurately document all patient interactions and interventions within HER systems.
- Contribute to ongoing initiatives to optimize care processes and meet practice and quality metrics.
- Schedule appointments, call patients with results or instructions, assist with medication refills and prior authorizations, and monitor EMR data consistency.
- Answer patient phone calls during business hours
- Medication reconciliation
- Serve as patient advocate, assessing care and ensuring quality care is given.
- Evaluate program processes and make recommendations to management that will improve the effectiveness and efficiency of the program.
- Assist with various projects as assigned by direct supervisor.

Requirements:
- Current Medical Assistant with a diploma or certificate showing successful completion of a course of study from an accredited institution that prepares Medical Assistants for their work
- At least 2 years of experience in a clinical or outpatient medical office setting.
- Strong understanding and experience in chronic care management or Nephrology care practices.
- Excellent verbal and written communication skills.
- Proficient in EHR/EMR systems and digital documentation.
- Comfortable handling high-volume patient caseloads in a structured, remote, or in-clinic setting.
Qualifications:
- Experience in nephrology or population health.
- Prior Care Management experience.
- Familiarity with billing and documentation requirements.
