Nurse Practitioners
Two Positions Available: 1 in the general Phoenix Metropolitan Area and 1 in the East Valley (Mesa) Area

As Nurse Practitioners, you will use your compassionate and empathetic nature to support the clinical care of patients with renal disease. You will work under the direction of the Director of Clinical Services and under the clinical supervision of the physicians to enhance and augment patient care.
As Nurse Practitioners, you should have:
- Master’s degree in nursing or related field, or equivalent combination of education and experience (4–10 years).
- Current Arizona RN license and Nurse Practitioner or Physician Assistant license.
- Proficiency in Microsoft Office and experience with ICD-9 and CPT coding.
- Solid interpersonal skills, flexible schedule availability, and reliable transportation for travel between sites.
Your essential duties and responsibilities as a Nurse Practitioner:
- Collaborate with AKDHC physicians and dialysis staff to complete and document dialysis visits, addressing medical issues promptly.
- Provide chronic kidney disease care in office settings and coordinate treatment plans with physicians.
- Support care transitions, including annual physicals and post-hospital discharge planning.
- Deliver care across settings (e.g., rehab, SNF, office, or on call), including diagnostic evaluations and treatment planning.
- Prescribe or recommend appropriate therapies and maintain professional, respectful conduct in all interactions.
